HIMSS describes it as the Continuity of Care Maturity Model. Created to help optimize outcomes for health systems and patients alike, CCMM outlines the progressive capabilities healthcare organisations need to possess in order to seamlessly coordinate patient care across a continuum of care sites and providers. This session will present a maturity model that addresses the convergence of interoperability, information exchange, care coordination, patient engagement and analytics with the ultimate goal of holistic individual and population health management.
